PRP therapy (platelet-rich plasma): Numerous Application Areas
The so-called PRP method is considered to be particularly gentle and promising, because it is treated with the human body's own growth factors. In PRP therapy, the body's own platelet-rich plasma is injected into the affected joint. This substance contains growth factors which initiate self-healing and regenerative processes in the injected area. These promising therapies, the PRP therapy just mentioned here and, in addition, the so-called hyaluronic acid therapy (Improvement of the visco-elastic properties of the diseased tissue) have numerous applications. For example, after cartilage operations, such as micro-fracturing, mosaicplasty or autologous chondrocyte transplantation, these healing methods are implied. Even after surgery on tendons like cruciate ligament, Achilles tendon or rotator cuff injuries, the treatments promise great success as tissue regeneration is stimulated and homeostasis in the joint improves.
Autologous Fat Transplantation
AMFAT (autologous micro-fragmented adipose tissue) therapy with Lipogems® is an innovative therapy in which the body's own fat is processed, micro-fragmented, purified and concentrated in a closed system. The liquid thus obtained is then injected into the affected joint. When applying this biological treatment, physicians take advantage of the fact that there are 500 to 1000 times more active, reparative cells (mesenchymal stem cells) in adipose tissue processed with this technique than there are found in the same quantity of bone marrow. Immunomodulatory and anti-inflammatory effects occur in the damaged joint treated with Lipogems. Different growth factors are released, initiating repair processes and restoring homeostasis (metabolic balance) in the joint. The processed adipose tissue acts like a biological reactor in the joint, reducing inflammation and initiating regenerative processes which support the body's own healing powers and protect the cartilage from further damage.
Cartilage Surgery: Progress or Stage Depending
Cartilage is a nerve-free tissue that is not supplied through the blood and therefore, is not capable of regeneration. This means, once cartilage damage has occurred, it cannot reverse or heal itself and keeps deteriorating. Leaving the condition untreated, full-depth cartilage damage leads to the development of premature joint degeneration, in other words to osteoarthritis. Deep cartilage damage usually requires surgery. Only then, a natural healing process can be evoked to restore the joint function. Depending on the size, location, and degree of joint damage, various procedures are available.
For small cartilage damage, the so-called micro- or now called nano-fracturing method is performed arthroscopically. It is a drilling into the bone lamella adjacent to the cartilage. Better results are now achieved with the additional application of a matrix. These cell-free implants serve as placeholders. They migrate into the cartilage cells and form regenerated cartilage.
If the cartilage damage is slightly greater or the underlying bone is damaged, cartilage bone cylinders can be embedded into the defective area from a peripheral zone.
Large articular cartilage defects can be successfully reconstructed biologically by means of cartilage cell transplantation. Here, autologous cartilage cells are removed from the body, grown in a special laboratory, and then implanted in the affected joint. This allows a hyaline, or hyaline-like, articular cartilage to once again grow, filling the defective area and possessing all the properties of the original cartilage.
Today’s modern methods are called matrix-associated autologous chondrocyte transplantations (MACT). It is important to note that concomitant injuries such as ligament instabilities, dislocation of the kneecap, and misalignments of the leg axis (X-leg or O-leg) must be treated concurrently in order to successfully proceed with cartilage therapy.
