About

Prof Dr med. Roman Ganzer has been Head Physician of Urology at the Asklepios Clinic Bad Tölz since July 2017. He is an acknowledged specialist in robot-assisted surgical techniques in the urogenital field through DaVinci. The entire spectrum of urology is covered in the Asklepios Municipal Clinic Bad Tölz. At the clinic, both conservative and surgical procedures are offered and carried out with a high degree of expertise on a daily basis. The department focuses on performing minimally invasive operations using the DaVinci system and 3D laparoscopy.

Faster rehabilitation thanks to DaVinci

Surgery with the DaVinci system is a state-of-the-art, continually developing technical advance that increases the precision of interventions to the highest degree possible. Using robot-assisted interventions that are primarily used in the urogenital field worldwide, the console operator uses hand and foot pedals to guide the arm of a "robot" which processes its movements precisely and jitter-free. The advantages of this technique lie in its outstanding imaging of the operating area through three-dimensional vision and the precise transmission of hand movements to the surgical instruments through seven degrees of freedom. This makes it easier to precisely prepare anatomical structures while not harming surrounding tissue. The operator's years of experience, therefore, lead to excellent results for the patient.

Minimally invasive interventions in the urogenital tract using DaVinci result in smaller cuts, reduced tissue trauma and generally low loss of blood, meaning that patients recover more quickly, require fewer painkillers and develop fewer complications.

Prostate - THE male gland

The prostate is an exocrine gland that produces part of a man's seminal fluid, and thus plays a part in a man's reproductive capability.

Changes to the prostate, whether malignant or benign, become increasingly frequent with age. Prostate cancer is the most common form of cancer in men. This form of cancer is often discovered early through preventive checks and responds well to treatment in its early stages.

Prof Dr Ganzer focuses on robot-assisted radical prostatectomy (complete removal of the prostate) using the DaVinci system. The procedure endeavours to preserve nerves (retain erectile nerves) whenever possible. Depending on the findings, the pelvic lymph nodes are extensively removed.

Focal therapy offers an alternative to surgery and radiotherapy when tumours of the prostate are detected very early and are well delineated. Focal therapy uses a variety of techniques to treat only the tumour, thereby protecting the surrounding tissue as much as possible. Following this procedure incontinence generally does not occur and potency is preserved. Focal therapy is still at the experimental stage and should therefore only be performed as part of studies. Prof Ganzer is an acknowledged expert in the field of focal therapy through high-intensity ultrasound. Prof Ganzer is the head of a nationwide study and a member of the German Urology Society's working group for focal therapy.

Organ-saving surgery in the event of renal tumours

Renal cell carcinoma (renal cancer) is the third most common urological tumour in men and the most common in women. If the tumour is discovered in time, patients generally benefit from an organ-saving procedure in which the tumour is removed at a safe distance while preserving the healthy portion of the kidney. Here, DaVinci surgery offers the advantage of the excellent delineation of the tumour from healthy tissue, as well as the option of keeping the ischaemia period (temporary interruption of blood supply to the kidney) short. Additionally, the entry trauma is significantly reduced as opposed to open surgery, and the patient experiences less pain and can be discharged relatively quickly. If a tumour is located very centrally within the kidney, it is often difficult to delineate it and only a partial removal (partial renal resection) is possible. In such cases, Prof Ganzer uses intra-operative ultrasound to aid delineation of the tumour.

Remedial action for changes in the lower urinary tract

Tumours of the bladder which, in their advanced stage, may necessitate removal of the bladder (cystectomy), can, in selected cases, be treated surgically using robot-assisted technology.

Congenital or acquired changes in the urinary tract (urethra) can lead to the defective discharge of urine from the kidneys. Affected patients often suffer from flank pains, as well as cumulative, severe and highly complicated urinary tract infections involving the kidneys. In order to prevent long-term damage to the kidneys, the patient must receive appropriate and timely assistance. Prof Ganzer routinely performs reconstructions of the urinary tracts (renal pelvis surgery, reconstruction of the urinary tract through psoas hitch and Boari plastic surgery) with the aid of DaVinci.
