No chance for a smoker's leg! - Specialist for peripheral arterial occlusive disease
Peripheral artery occlusive disease (PAOD, intermittent claudication, "smoker's leg") is a chronic degenerative vascular disease in which the arteries are progressively becoming occluded at a slow pace, especially in smokers. The symptoms suffered by the patients include mainly pain with longer walks, which can occur even with shorter distances or during rest, as the disease progresses. In the late stage of the disease, due to the diminished blood flow to the extremities, especially the legs, skin changes occur, leading all the way to necroses. These necroses heal badly and oftentimes the patient's foot must be amputated step by step ("the smoker's leg"), since the damage cannot heal properly due to the lack of blood circulation. The vascular-surgical specialists can support the patient at an early stage in order to break the vicious circle of the reduced blood supply, for example, by means of a bypass (artificial bypass circulation). In addition, special attention is paid to the treatment of people with diabetes and vascular problems (the so-called "diabetic foot"), whereby carrying out complex bypass operations on the lower leg or foot (the so-called pedal bypass), together with the intensive internistic diabetological care, occupy a prominent position. In addition, a highly specialised therapy of chronic wounds/diabetic foot syndrome is achieved by a certified wound management using the most modern therapy forms. Certified aortic centre - therapy at the highest scientific standard
Furthermore, Prof. Dorweiler has mastered the most modern procedures for the treatment of diseases of the aorta and of the large abdominal vessels. Frequent disease symptoms at this location are, for example, aneurysms or pathological narrowings (e.g. aortic stenosis, renal artery stenosis, visceral artery stenosis). Aneurysms are irregular dilatations of the vessels that can lead to thrombosis and embolism (e.g. leg embolism, stroke), or in case of lacerations, they may lead to massive and usually fatal bleeding. Depending on the position of the vessel alteration, open operations (with an abdominal or a peripheral cut) or endovascular methods can be used. In the case of endovascular technique, an intervention is usually performed via one or both inguinal arteries under local anaesthesia and the work is performed via special catheters in the so-called "hybrid operating room", which is equipped with the latest angiography technology.
The Department of Vascular Surgery has been successfully certified annually since May 2010 as an aortic centre, in accordance with the Declaration of Conformity for quality assurance of abdominal aortic aneurysm (G-BA) by MDK (Medizinischer Dienst der Krankenversicherung, Medical Service of the Health Insurance Funds) Rheinland-Pfalz.
Interdisciplinary Care of Multimorbid Patients
Internal medicine patients who require dialysis because of chronic renal failure, need a so-called "AV fistula" or a dialysis catheter for this treatment. When dialysis is required due to acute renal failure, dialysis catheters such as Shaldon or Demers are implemented. These large-lumen accesses lie in a large body vein or in the right atrium and are not suitable for a permanent application in view of the risk of infection. If the dialysis is to be continued for an extended period of time, the patients are given a dialysis shunt by the vascular surgeons. This shunt is necessary to guarantee safe access for the dialysis treatment which is crucial for the patient's survival. The AV fistula (dialysis shunt) is an artificial connection between the artery and a vein ("AV") and it is usually applied to the patient's forearm.
Prof. Dorweiler and his team also treat constriction of the carotid artery ("carotid stenosis"). Carotid stenosis is especially dangerous, as it can lead to reduced blood flow to the brain and thus to a stroke. Nowadays, such vascular changes can be well diagnosed by vascular ultrasound or angiography. The narrowing is caused by excessive calcifications. These can be removed by the vascular surgeons over a course of a short operation (about 90 minutes).
