Gastroenterology - a Broad Specialty
Gastroenterology is concerned firstly with diseases of the gastrointestinal tract, liver, gallbladder and pancreas and other autoimmune diseases such as autoimmune hepatitis (AIH), autoimmune pancreatitis, ulcerative colitis or Crohn's disease. Patients from young to old may suffer from acute or chronic gastrointestinal diseases. These include for example celiac disease (gluten intolerance), which usually begins in childhood, Crohn's disease or ulcerative colitis usually make a first appearance in young adults, and colon cancer is more of an illness seen in patients above 60.
In particular, inflammatory bowel disease (IBD), Crohn's disease and ulcerative colitis are common medical conditions that can also lead to systemic damage, including the joints alongside the intestinal changes. As ulcerative colitis in particular is associated with an increased risk of developing colorectal cancer, patients with this disease are urgently advised to undergo regular colonoscopies.
Endoscopic Examination: Gastroscopy and Colonoscopy
Endoscopic examination (mirroring) is done using a flexible examination apparatus with camera. In general, stomach endoscopy (gastroscopy, EGD, esophagogastroduodenoscopy) is differentiated from a colonoscopy (colonoscopy). The examination is usually under mild sedation (half asleep), so the patient feels no pain during endoscopy. Gastroscopy is mainly used for the assessment of changes in the esophagus, stomach and duodenum. Here, inflammation, ulcers, tumours or mucosal changes (e.g.: due chronic heartburn) can be shown. During an endoscopic examination, biopsies (tissue samples) of suspicious areas can be taken.
During a colonoscopy which always precedes a colon cleansing, attention is paid mainly to signs of inflammation or polyps. As polyps can transform malignantly to cancer, they are removed during a colonoscopy. This non-painful procedure should be carried out prophylactically for all patients over 55 every 10 years, because the removal of polyps can prevent colon cancer, which may result from these growing polyps.
Bowel Cancer - Colorectal Carcinoma, Colon Carcinoma
At 95%, colon cancer is one of the most common tumours of the gastrointestinal tract. Colorectal cancer is the second most common cancer in women and men. As colon cancer usually causes symptoms only at a very late tumour stage, screening tests, such as colonoscopy are essential for early detection. Since colon cancer is always created from previously benign polyps (= mucosal growth), an emerging tumour can be "cured" with polyp removal.
