Coronary heart disease (CHD) is a degenerative change in the coronary vessels that advances over the years and is caused by risk factors such as diabetes mellitus, nicotine abuse or excessive blood fat levels. This change of the heart vessels can lead to constrictions resulting in shortness of breath (dyspnoea) or stabbing pain during exercise or even at rest.
State-of-the-Art Non-Invasive and Invasive Cardiac Diagnostics
Diagnosis of CHD can be performed using non-invasive techniques. A resting ECG (electrocardiogram) or ergometry (stress ECG) can provide first indications of the disease. Furthermore, there is the option of imaging using cardiac computer tomography (cardiac CT) or cardiac magnetic resonance imaging (cardiac MRI).
Invasive diagnostics include coronary angiography, an imaging procedure that physicians apply by using contrast agents to visualise the lumen of the vessels. The examination is performed through a left heart catheter. This examination technique involves an experienced surgeon who pushes a catheter depicted by a contrast agent through a small skin incision in the brachial artery or leg artery into the coronary arteries.
The procedure is performed under local anaesthesia and sedative medication; however, the patient can breathe independently. If the vessel lumen is constricted, balloon dilation is performed and a stent is implanted to provide internal splinting of the constriction.

One of the most common cardiac arrhythmias is atrial fibrillation, which can occur both in sudden attacks and persistently, leading to a too fast and irregular heart rate (tachycardia). As a result, the patient experiences a lack of resilience, shortness of breath during exertion or chest pain.
In addition to immediately fine-tuning individual medication for the patient, which primarily serves to prevent an embolic event, the Helios Clinic Munich West offers an interventional procedure as a therapy option.
If antiarrhythmic drug therapy is not successful, a so-called ablation method can be applied by physicians. They insert special catheters during the electrophysiological examination (EP study). In the meanwhile, high-frequency current is emitted resulting in changes of the ability of heart muscle cells to convey capacity, thus treating the cardiac arrhythmia. Another method is sclerotherapy in which doctors use the so-called cold cryoablation. This procedure is carried out exclusively by experienced rhythmologists such as Prof. Reithmann and Dr Ulbrich and promises good therapeutic success alongside very few complications.
Another special focus of the department is the ablation of cardiac arrhythmias from the heart chambers (ventricular extrasystoles and ventricular tachycardia).
