Minimally Invasive Urinary Incontinence Operations

Urinary incontinence is the unintentional loss of urine. A distinction is made between urge and stress incontinence. If incontinence cannot be corrected by either drug therapy or pelvic floor training, it can be treated surgically by experienced specialists. If stress related incontinence is diagnosed, the surgeons use laparoscopic colposuspension. This is a minimally invasive procedure performed using camera technology (endoscope) without foreign tissue.

Endoscopic Incontinence Surgery Applying the Burch Method

If endoscopic operations at the abdominal cavity are performed simultaneously, lateral bladder elevation (Burch colposuspension) is applied. This technique is minimally invasive as well and does not require the insertion of a plastic band.

With Burch colposuspension, doctors create an artificially filled air space between the pubic bone and the bladder. With the aid of camera optics, the surgeon has a direct view of the bladder and the surrounding vaginal roof.

In order to restore the pressure of the bladder and thus its ability to close, the vaginal roof is slightly raised by means of non-dissolvable threads next to the urethra. This procedure is also minimally invasive, and patients are able to leave the clinic after a few days if there are no complications.

Correction of a Vaginal Prolapse

A lateral defect can lead to a protrusion of the bladder into the anterior vaginal wall (cystocele) which is usually asymptomatic at first, but can then onset in form of recurrent urinary tract infections, urinary retention or urinary incontinence. Detailed gynaecological diagnostics are used to determine the exact disorder and then a plan for an optimal therapy is put in place.

In the case of a so-called pulsion cystocele, where the defect occurs in the area below the bladder, anterior vaginoplasty is performed.

The specialists in Dormagen are also able to treat this bladder deficiency without opening the vaginal skin. The thinned tissue is tightened from the abdominal cavity and a stronger layer below the bladder is thus constructed. The specialists have developed a special endoscopic access procedure to correct lateral deformity without using foreign material, eliminating the need for an abdominal incision. 

Endoscopic Prolapse Surgery

Due to an additional lack of suspension of the pelvic floor as a whole, not only can a cystocele occur, but also a prolapse of the entire vagina along with the uterus can be a result. In order to counteract this downward prolapse, the so-called sacropexy and pectopexy methods are performed, generating a pull upwards. Here, experienced surgeons attach a plastic band to the sacrum or to the side of the pelvis and attach the pelvic floor to it.

This operation is performed laparoscopically and spares tissue, so the vaginal axis is not distorted. These specially applied techniques make it possible to correct prolapse without scarring the vagina and thus provide excellent treatment results. Since pectopexy has advantages over classical sacropexy, this is the procedure now predominantly used in hospitals.
