This memorable quote by the American nephrologist Max Gallo illustrates that a thorough examination of the kidneys often provides insight into a complex systemic disease. Dr Jan Brügger takes this quote to heart because he does not simply treat kidneys in his specialist practice but first and foremost treats people with their kidney diseases.

State-of-the-Art Dialysis Procedures: Hemodiafiltration (HDF), Hemodialysis (HD), and Hemofiltration (HF).

The modern equipment allows Dr Brügger to offer customized therapy for the individual needs of his patients.

In conventional hemodialysis, the blood flows along with a special fluid (dialysate) and is separated from it by a fragile membrane. In this process, all those substances that the organism wants to excrete flow from the blood across the partially penetrable membrane into the dialysis fluid due to differences in concentration. In exchange, minerals and glucose are added to the bloodstream to keep the electrolyte balance and blood glucose levels balanced. This automatic process, which slowly equalizes the concentration differences between blood and dialysate, is called diffusion. Since the blood has to circulate through the machine several times during a dialysis session lasting several hours for this purpose, conventional hemodialysis is the most time-consuming renal replacement procedure.

Hemofiltration is particularly indicated if therapy-resistant high blood pressure occurs during dialysis treatment. In this procedure, the urinary substances are not removed from the patient's blood by diffusion as described above but are forced through a filter by applying high pressure. In this process, the cellular components of the blood (e.g., red/white blood cells) do not pass through the filter membrane and remain in the blood. Small molecules, urinary toxins, and water, on the other hand, are filtered out. An accompanying infusion must compensate for losing these vital substances into the filtrate. The high fluid filtration in this blood washing procedure counteracts the development of high blood pressure.

Hemodiafiltration is a combination of hemofiltration and hemodialysis. It combines the advantages and disadvantages of the two methods described. As a result, the total amount of toxins eliminated from the blood is more significant. Therefore, this procedure is indicated when conventional hemodialysis is no longer sufficient for effective detoxification.

Specialist for Peritoneal Dialysis

Peritoneal dialysis uses the biological filtering properties of the peritoneum to save the dialysis machine. This makes this method easy to use at home. This form of renal replacement therapy works on the same principle as hemodialysis. The dialysate enters the abdominal cavity through an inserted catheter. It absorbs urinary toxins from the blood via diffusion through the peritoneum membrane before they can be drained from the abdominal cavity.

Since fluid loss is slower with peritoneal dialysis, this procedure is gentler but more ineffective and must be performed daily at home.

High Level of Expertise in the Field of Special Blood Purification Procedures

In addition to conventional renal replacement therapy, Dr Jan Brügger, a nephrology specialist, also has many years of experience in dealing with other blood purification procedures (apheresis). For example, lipid apheresis is used to remove harmful triacylglycerides (esterified fatty acids) from the blood in cases of elevated blood lipid levels to effectively reduce progressive vascular calcification and thus the risk of heart attacks and strokes.

Protein apheresis also purifies the blood of fat and cholesterol, which circulate in the blood bound to lipoproteins (proteins). This can effectively lower high levels of harmful LDL cholesterol and alpha lipoprotein that are resistant to therapy.

Diagnosis of Kidney Diseases

In his specialist practice, Dr Jan Brügger has all the necessary equipment to diagnose kidney diseases reliably using high-resolution ultrasound. This makes it possible to describe the extent of inflammations of the kidney tissue (nephritis) to recommend an optimal therapy. A sonographic examination can also clarify painful obstructions of the urinary tract caused by kidney stones. The therapy recommendation here depends on the size and localization of the kidney stone.

Comprehensive Medical Preliminary Work-up and Follow-up Care for Kidney Transplantation

A time-consuming dialysis therapy usually bridges the waiting period until a suitable donor kidney is available.
