The team led by Dr med Alfred Tylla focuses on reconstruction and preservation of function after injuries to the knee joint. The certification as a center for knee surgery in Bavaria accredits this department with exceptionally high case numbers for all interventions around the knee joint, which add up to 600 interventions annually. Patients, but also referring colleagues, benefit from this enormous experience level.

Regenerative Cartilage and Joint Surgery: Use of State-of-the-Art Therapeutic Procedures

At Rummelsberg Hospital, in addition to established procedures of conventional knee surgery, pioneering achievements are also performed, advancing medical research. Such offers are always understood as alternatives to techniques that have been tried and tested for a longer period and are only included in the hospital's range of services after careful research. For the treatment of minor cartilage defects, microfracturing has also proven to be a beneficial concept. In this procedure, tiny fractures are created in the degenerated cartilage using drilling holes, so that stem cells can migrate there, from which a new cartilage cover can develop.

In the case of larger cartilage lesions, cartilage cell transplantation is also an option. In this case, healthy chondrocytes (cartilage cells) are removed in a first procedure, cultivated in the laboratory, and transplanted to the defect site in a second procedure. These and other innovative therapeutic approaches for joint defects are routinely used by Dr Tylla and his team in Rummelsberg.

Specialists in Trauma and Sports Injuries: Treatment of Meniscus Tears and Ligament Injuries.

Competitive athletes and averagely active persons are afflicted by severe knee traumas involving the menisci and the ligaments in everyday life. These injuries often take a long time to heal on their own. Still, it is not uncommon for the joint's stability to be permanently impaired afterward and for repeated dislocations.

To avoid such unpleasant experiences, it is recommended to reconstruct the natural conditions in the joint by ligamentoplasty to ensure sufficient ligament tension and, consequently, stability. Postoperatively, physiotherapy enables patients to secure the treatment result with the moderate exercise of the stabilizing musculature.

Diagnosis and Therapy for Patellar Instability

An awkward movement in everyday life is often enough to cause the patella to jump out of its bony guide. Usually, the small bone can be easily repositioned, but the joint still swell painfully. A shallow bony groove makes some people particularly predisposed to this injury, and it is not uncommon for the stabilizing medial patellofemoral ligament (MPFL) to be affected as well. Ligamentoplasty is advised if this is the case. This involves harvesting a tendon from the patient's body (gracilis tendon in the front of the tibia) to replace the ruptured ligament. In this way, a new dislocation of the patella can be effectively prevented despite anatomical susceptibility.

Correction and Surgery of Leg Axis Misalignments (Bow- and Knock-Knees)

All lower extremity axis abnormalities accelerate wear of the articular cartilage and can result in severe osteoarthritis of the knee. These axial abnormalities can be acquired through injury or genetics; by performing a realignment osteotomy, Dr Tylla can straighten the leg axis by wedge-shaped bone excisions and subsequent screw fixation with angle-stable titanium plates to distribute the load evenly over the joint surface. This can prevent or at least slow down cartilage damage. In addition, due to the immediate relief of already damaged cartilage, many patients report significant relief of their pain.

Expertise in the Treatment of Mild Cartilage Damage / Early-Stage Osteoarthritis

Any incipient osteoarthritis is in urgent need of treatment, even with mild pain symptoms. Early intervention can relieve the joint cartilage, an intervention that has a more effective and lasting effect than any pain medication. In addition, through timely diagnosis, a small arthroscopic intervention (joint endoscopy) can often avoid the costly implantation of a total endoprosthesis in an open surgical technique. In the case of arthrosis, painful waiting always means a loss of quality of life and a worsening of the symptoms due to increased wear and tear.
